Now that we don’t have a nanny, we’re probably going to have to rely on babysitters from time to time, so I whipped up this Emergency Contact Sheet that we’ll keep on our refrigerator at all times. Well I came up with the content, but our super talented Creative Director Minhee designed it (more on her in a coming post!). Download a printable pdf of the Emergency Contact Sheet here!

I’m glad you included allergies. I babysit for a 10-month-old occasionally and didn’t know until my third time watching him that he has an oat allergy!

This is so handy. My husband and I took a trip over the summer and I had to put something like this together.
I also printed out a form to authorize my mom (who was babysitting) to make medical decisions since my husband and I were going to be out of the country. Not necessary for a night out, but important for a long trip.

We haven’t hired a sitter, ever, but I will definitely use your form if and when we do!

Love this! So useful. Though we have yet to have a sitter who’s not family.

This is awesome! Even with family watching the kiddo, it’s nice to have the sheet handy for the police/fire contact info. It’s hardly something you normally have memorized. As well as any other emergency procedures. Will need to print and laminate this for my fridge!

Great printable. We needed something like this and hadn’t gotten around to it so this was very useful.
