I have two kids in diapers but only one diaper bag! It’s a lot more streamlined than it used to be, and I try to pick items that take up less space (spare leggings instead of spare jeans for example).
1) Change pad by HipPadBaby. It’s just a little waterproof and absorbent pad big enough to fit under the bum. I like that I can change the girls anywhere with it, even in a carseat!
2) Avent soothers. I have always have a big one and a little one for naps.
3) Spare change for the baby. Either a onesie or a sleeper depending on the weather.
4) Spare pants for the toddler.
5) Diapers! In size 2 and 4. Usually I have Huggies Pure and Natural, but I like G-diapers when I can find the inserts (they don’t sell them in town) and sometimes I throw in cloth diapers if I’m out of disposables.
6) Spare shirt for me. I have a spitty, spitty baby.
7) Little muslin burp cloth blankets. Not these ones (mine are from Chile) but it’s the same idea. Again, spitty baby.
8) Rubbermaid Litterless Juice Boxes. For water for Toddler Girl. It’s the only sippy cup she ever liked and still very handy for when we are out.
10) Wipes. I actually don’t have a favourite wipes brand. I use cloth wipes at home and just buy whatever I happen to find when I run out.
11) The Ginger bag by Epiphanie. Mine is dark brown with a teal interior, but you can’t get it anymore. It’s not a diaper bag, it’s a camera bag. I find that most diaper bags look like diaper bags, which I wasn’t so into. This bag has lots of room, handy dividers, and in a couple years is going to be an awesome bag for my camera! If I were to do it again, I would get one that is more on the tall/slim end than the short/fat end as it’s not so great with the stroller (we didn’t use a stroller for the first year with TG). It usually lives in the car or gets taken to people’s houses.

I carry similar items though being a dad I do no carry an extra shirt for myself. If the baby spits up on me, so be it! But I am not a big fan of shoulder strap bags and only one compatible makes backpackers that are actually designed as diaper bags, diaper dude! Mrs. Jacks got me one as a dads gift when number 2 was born and I love it!

loove the bag! I’m surprised you don’t cloth diaper out and about. My bag is always so full from cloth dipes! haha So my diaper bag looks very different; cloth dipes, cloth wipes, wet bag, and wipes solution.

@looch: TG has books in the car, and a stuffie, but they stay in the car. She honestly doesn’t need much entertaining. The baby is just starting to need toys, so Sophie will start traveling with us soon!
@Coco Bee: if my girls sit in a wet diaper for more than 5 sec they rash, so sposies work better when we are out. Sad but true.

So many of my friends cram their diaper bags full of STUFF, then have a really tough time finding things or lugging the 500 lb. bag around. Really love this streamlined bag rundown, especially the camera bag idea!
