I follow a twin message board on another parenting website, and one of the most common questions I see is, “Could I be pregnant with twins?” So many people ask this based on a variety of facts that they think may make them more likely to be experiencing a twin pregnancy.
The short answer is only an ultrasound will tell, and you just have to wait and see. No one is ever really satisfied however with that answer, so I thought I’d address some of the common reasons people think they might be pregnant with twins.
1. I got pregnant via [specific fertility treatment – Clomid, IVF, etc.]. There is certainly an increased chance of twins depending on the treatment you used. Hopefully, your RE or OB has discussed the increased “risk” of multiples and you’re fairly prepared. At the end of the day, you still have to wait for your ultrasound, but hopefully the close monitoring at least results in an ultrasound earlier than you might have otherwise! We found out at our 8-week ultrasound.
2. I/My husband have/has twins in the family. First, there is no medical evidence that identical twins “run” in families; science can’t yet explain why an egg splits. There can be a hereditary link to fraternal twins in the maternal side of the family. Sorry, but everything I’ve read indicated that twins in your husband’s side of the family doesn’t help (though it might increase your daughter’s chances of having twins!). If you have fraternal twins in your family, you may have inherited the gene for hyper-ovulation, making you more likely to release more than one egg at a time. For example, both of my grandmas are twins. My maternal grandma is an identical twin who gave birth to fraternal twins. The fact that she’s an identical twin doesn’t make me any more likely to have twins (although, I did have identicals as it turned out!). The fact that she also gave birth to fraternal twins, however, does make it slightly more likely that I could give birth to a set of fraternal twins. My paternal grandma was a fraternal twin. If her mother had the gene for hyper-ovulation and passed it down to her, there’s a slight chance that my father (who it obviously would not have affected) could have passed that to me. Is it likely that I’ll have fraternal twins? Nope. Yes, the fact that there are fraternal twins in the family makes it ever so slightly more likely, but not a sure thing.
3. I’m soooo tired and/or having a ton of morning sickness–way more than I had with my other pregnancy. I think most twin moms experience a lot of fatigue all through pregnancy, but there’s really no way to tell whether you’re extra tired because you’re pregnant with twins or just extra tired because you’re pregnant while chasing another kid. This is a completely different pregnancy, you’re a little bit older, or a variety of other reasons. Like the fatigue, some twin moms experience pretty terrible morning sickness. I had a pretty awful round of it myself, but one of my best friends who was pregnant with one baby had a much, much, much worse case of morning sickness. Being tired and having a lot of morning sickness are two of the most common reasons people wonder if they’re having twins, and there is really just no way to tell. I really don’t think these symptoms indicate an increased chance that you’re pregnant with twins. Every pregnancy is different, and these are just not sufficiently reliable indicators to put any weight on in trying to guess how many beans you have growing in there.
4. I just know I’m pregnant with twins/I keep having dreams I’m pregnant with twins. There may be something to this one. I had absolutely no clue I was pregnant with twins. I hadn’t even considered the possibility, even though I had twins in the family and had spent a good part of my youth praying for twins. I was also pretty sure we were having girls before our gender scan, which was obviously false, so clearly I just have no mommy intuition. While it didn’t work for me, a lot of twin moms report having had some intuition, dreams, etc. that they were pregnant with twins. On the other hand, I may or may not know of a couple who remained convinced they were having twins even after an ultrasound showed one baby and they definitely had only one baby, so I would hold off on buying two cribs until you get that ultrasound with two lil’ tadpoles!
5. My HCG levels are high. Unfortunately, this also really isn’t a reliable factor to depend on for multiples, though it’s true that multiples can lead to higher numbers. Some people have higher HCG levels with their singleton than they did with their twins. It’s certainly something to consider, but it’s like being extra tired or having a lot of morning sickness….It’s just not something you can put a lot of weight on without more information. High HCG levels could be because you’re having twins or it could just be that this pregnancy is unique!
Much like the wait for a positive pregnancy test, if you think you might be pregnant with twins, there’s really nothing to do but wait, wait some more, and then keep waiting for that ultrasound! For fun, however, you can find the early pregnancy symptoms I had with my twin pregnancy in this post!

I had absolutely no clue I was pregnant with twins until our first u/s. That was at 9 weeks and man, once that image popped up on the screen, it was SUPER clear there were two babies before the tech said a word. Silly me, I tried to talk myself down and wait for the tech to say, “there’s the baby and there’s something that looks like a baby, but it’s not a baby.” That so did not happen, haha! That is a birthday I will never, ever forget.
In hindsight, the fact that I was already busting out of my pants at 7-8 weeks could have been a clue that I was pregnant with twins, but at the time, I just thought I was starting to show early. Ha!
Re twin running down the father’s side of the family, this is what I always tell people: your husband’s sperm cannot affect how many eggs you ovulate, so it doesn’t matter if twins run in his family or not.

We conceived with Clomid for both of our viable pregnancies, and both times I released two eggs. My hCG appropriately rose with Chloe, but this time it quadrupled at each blood draw (and I have 4 draws). My doubling time + two eggs made my RE wonder if we were having twins, and we were absolutely preparing for it. My 6w ultrasound showed that we have one baby, though!

My HCG levels were so low that the nurse initially told me I was not pregnant! Then a blood test confirmed pregnancy. I had horrendous morning sickness including a trip to the ER for dehydration at 8 weeks and found out it was twins at 9 weeks.

Very interesting post, especially on the genetics explanation! DH would roll his eyes at the dream/intuition comments moms make though. hahaha
In Canada, doctors will not allow (or highly discourage) the insertion of more than 1 embryo if there’s a good chance that you will eventually get pregnant (young, healthy) to avoid the higher chances of having twins/triplets!

I had no clue, I thought they were going to tell me there was no heartbeat. The NP tried first just with a Doppler and couldn’t get a clear beat so she went to get an ultrasound machine, then she got their weird look on her face and went to get the ultrasound tech, then they both sat there saying do you see what I see? are you sure? Yep I see it? I was freaking out because I thought it was a miscarriage. Then they turned the screen to me and said it was twins. Then I freaked out. I had no clue or inkling that I was pregnant with twins but it was early (6weeks) so I did get quite a few symptoms after that. Lots of morning sickness and showing early were my big signs.

be careful. I jokingly told my sister (pregnant with her third child and desperately hoping for a girl) that she would have twin boys instead. It backfired a bit on me and I got the surprise pregnancy with twin boys.

This is very interesting! I conceived Liam on clomid and my hCG levels were really high, which made me think it could be, but it wasn’t.

We used clomid and my hCG numbers were high on my 3 day blood draw post IUI. We found out at my ultrasound 2 weeks post IUI that there were 2 sacks. I saw the screen and knew what I was looking at so when the tech said two I was already looking at my husband to see his reaction to the tech. It was priceless. You never could guess how fast someone could literally lose all the color in their whole body.

Thank you for mentioning the HCG thing! I had very high HCG levels from the very beginning with this pregnancy and only have a singleton so I know it can just happen that way. Sometimes I see people with high HCG levels getting super excited that they must have twins/triplets and I don’t know how to tell them that the only reliable way to tell is an ultrasound…

My mom had fraternal twins (my little brother and sister) in 1983, before ultrasounds were so routine. She didn’t find out until she was 6 months along that it was twins — although she says she suspected it because she was so much bigger than she had been with me or my other brother!

@saroy: my DH has a twin sister and my MIL didn’t find out until 2 weeks before she gave birth at 40 weeks! SIL was a big baby, so they just thought it was another big baby. so crazy!

My dad is a fraternal twin and also has fraternal twin brothers, he’s one of 10! I knew I had a chance of fraternal twins but was completely surprised at our 1st appt when we found out it was identical twins. Since they were my 1st I don’t know if I had more symptons than a singleton pregnancy. But now that I had my daughter ( you can bet we were quite anxious for our 1st ultrasound) I think I was probably a bit more tired.

My grandmother didn’t know she was having twins until she had delivered my mother and the midwife said, here comes another one! It was her fifth pregnancy and she had felt bigger and more tired, but she had put it down to being older. I grew up thinking that finding out you’re having twins while giving birth thing was a thing of the past, until I worked with a woman who told me the same thing happened when she was born in the late 70’s!
